Coordination of Hospital Care/Coordination of Subspecialties/Crisis-Care Management and Support
When hospitalization is necessary, Dr. Maldonado will personally arrange for your admission and coordinate your care while you are in the hospital. When necessary he will communicate with other physicians and/or family members locally or out-of-state, who are involved in your medical care. When appropriate, Dr. Maldonado will arrange for a direct admission to the hospital thus avoiding a wait at the Emergency Room.

If you are in need of a subspecialty referral, our staff will personally arrange for an appointment and follow-up visits with other physicians and will coordinate the care with these physicians. The staff will help you to schedule appointments at testing facilities when necessary.

Prescriptions Facilitation
For your convenience, Dr. Maldonado will arrange for same-day prescription refills.

The staff is available to assist you on the process of prescription refills and will answer any questions that may arise during this process.

Note: The exact amenities and services for which you will pay the annual fee are set forth in the membership agreement you will sign to become a member of the Practice.
